Our team of physicians and nurses will work directly with patient’s families to determine which medical supplies and equipment the patient needs to improve quality of life and keep them comfortable throughout our duration of care. Custom Hospice will handle the responsibility of ordering the necessary products and coordinating delivery to the patient’s home in a timely manner.
